A 38-year-old black man with a history of tobacco dependence was admitted to the Medicine Service with a history of 1 month of progressive dyspnea on exertion. Vital signs on presentation were within normal limits; his comprehensive physical examination was unremarkable except for the presence of bilateral white corneal opacities (). His electrocardiogram on admission showed normal sinus rhythm with no concerning ischemic changes. Serial cardiac biomarkers were negative. The patient was found to have a measured low-density lipoprotein level of 392 mg/dL. He was started on high-dose rosuvastatin and referred to a lipidologist for further management.
