Stroke risk high for patients undergoing aortic valve repair

Stroke occurrence after aortic valve surgery appears to be higher than expected, researchers reported at the International Stroke Conference 2013. The risk for stroke complicating aortic valve repair is estimated to range from 2% to 10%; however, outcomes have not been well documented, according to Steven R. Messé, MD, assistant professor of neurology at University of Pennsylvania, and colleagues.

Monitoring cholesterol, other parameters urged for children with HIV

Children with HIV are at an increased risk for lipid abnormalities and insulin resistance, and vigilant monitoring of these abnormalities is necessary to provide treatment, according to findings published online recently.Rohan Hazra, MD, of the Eunice Kennedy Shriver National Institute of Child Health and Human Development at NIH, and colleagues published data on 249 children in Latin America with perinatal HIV. The researchers looked at the children’s insulin, lipid and glucose levels.

Bariatric surgery improved cardiac abnormalities in obese adolescents

In extremely obese adolescents with previously undiagnosed cardiovascular abnormalities, researchers from The Ohio State University College of Medicine and Nationwide Children’s Hospital in Columbus found that bariatric surgery was beneficial. According to a press release, more than half of the adolescent patients’ cardiovascular abnormalities were similar to those of a middle-aged patient with cardiovascular disease.Researcher Marc Michalsky, MD, of the department of surgery in the division of pediatric surgery, and colleagues conducted a retrospective analysis of 10 female adolescent bariatric surgery patients with a mean age of 17.4 years and BMI of 50.33 kg/m2.

CMS issues final rule on Physician Payment Sunshine Act

CMS recently announced in a press release its final rule on the National Physician Payment Transparency Program: Open Payments, also known as the Sunshine Act, aimed to increase transparency among physicians, drug and device manufacturers and health care providers. As part of the Affordable Care Act, the rule calls for manufacturers of drugs, devices, biologics and medical supplies reimbursed by Medicare, Medicaid or Children’s Health Insurance Program to disclose payments made to doctors and teaching hospitals to CMS. According to the release, these manufacturers and group purchasing organizations must also report physician ownership and investments in these enterprises. This data will be publicly available on a CMS website by September 30, 2014, and entities required to report information under the rule will be able to review and correct reported data before it is released to the public. Collection of this information will begin August 1 of this year, according to the release.

S. pneumoniae prominent among bloodstream infections in sickle cell disease

Despite widespread availability of penicillin prophylaxis and pneumococcal vaccines, Streptococcus pneumoniae was observed to account for the largest percentage of bacterial pathogens in bloodstream infections among children with sickle cell disease.Previous studies have identified S. pneumoniae and Haemophilus influenzae as the leading pathogens responsible for invasive bacterial infections among the pediatric sickle cell disease population. However, after the introduction of penicillin prophylaxis and conjugated Haemophilus influenzae type b and pneumococcal vaccines, the incidence of bacterial infections in the pediatric sickle cell disease population has significantly decreased.
