GLP-1 therapy shows promise for severely obese adolescents

Despite recent FDA approvals for the adult population, obesity therapies for adolescents are limited. However, preliminary data from a 3-month trial examining the effects of a glucagon-like peptide-1 receptor agonist on BMI in severely obese adolescents have yielded positive findings. Researchers conducted a 3-month, double blind, randomly assigned, placebo-controlled, multicenter trial followed by a 3-month open-label extension trial. Patients (n=26) aged 12 to 19 years with severe obesity were enrolled and received lifestyle modification counseling and randomly administered exenatide (Bydureon, Amylin Pharmaceuticals) or placebo injection twice daily.

QUEBEC CITY — Careful selection is imperative when identifying adolescents who are candidates for bariatric surgery, according to Laurent Biertho, MD, who presented on this topic at the Canadian Pediatric Endocrine Group 2013 Scientific Meeting.Adolescents who are selected for bariatric surgery have a BMI of 40 kg/m2 or more or a BMI of 35 kg/m2 and a comorbidity such as diabetes or hypertension, Biertho, who is clinical professor in the department of surgery at Laval University, Quebec City, said in an interview with Endocrine Today.

Adolescents had varied beliefs about the underlying cause of their juvenile arthritis, with genetics and the immune system being the most common, according to study results.“The most important thing we learned was that beliefs about the cause of arthritis among these young people were varied, related to symptoms at presentation as well as the route of referral,” researcher Kimme Hyrich, MD, PhD, FRCPC, honorary consultant in rheumatology at the University of Manchester, told Healio.com.

On Feb. 5, the Congressional Budget Office (CBO) reduced its estimate of the cost to freeze Medicare payments under the physician fee schedule (PFS) at current levels by more than $100 billion. Medicare spending under the PFS is driven in large part by the sustainable growth rate (SGR) formula. Under the current formula, the CBO projects that Medicare payments to physicians will be reduced by about 25% percent in January 2014 before increasing by small amounts in subsequent years.
