Stroke risk high for patients undergoing aortic valve repair

Stroke occurrence after aortic valve surgery appears to be higher than expected, researchers reported at the International Stroke Conference 2013. The risk for stroke complicating aortic valve repair is estimated to range from 2% to 10%; however, outcomes have not been well documented, according to Steven R. Messé, MD, assistant professor of neurology at University of Pennsylvania, and colleagues.

Atrial fibrillation of any type was associated with more than a twofold increased risk for all-cause and CV mortality among patients undergoing transcatheter aortic valve replacement at 1 year, study results found.To assess the effect of AF on clinical outcomes among patients undergoing TAVR, researchers enrolled 389 high-risk patients into a prospective registry from August 2007 and October 2011.

PFO closure bested medical management alone for reduction of ischemic stroke

Patients with a history of cryptogenic stroke had better outcomes with patent foramen ovale closure as compared with medical management alone, according to data from the RESPECT PFO trial.Results from the overall trial, presented previously, demonstrated that PFO closure with the Amplatzer PFO Occluder device (St. Jude Medical) reduced the frequency of recurrent ischemic strokes, in comparison with medical management alone consisting of antiplatelet medication or warfarin. The trial included 980 patients (mean age, 46 years). Researchers performed detailed patient screening to exclude cardioembolism, large artery arteriopathy, small artery disease and other non–PFO-related stroke causes.At the International Stroke Conference 2013, Jeffrey L. Saver, MD, director of the stroke and vascular neurology clinics at Geffen School of Medicine, University of California, Los Angeles, and colleagues presented data on qualifying and outcome ischemic strokes in the RESPECT PFO trial.

Two studies challenge initial findings of iFR vs. FFR

Two studies published in the Journal of the American College of Cardiology were unable to clinically substantiate instantaneous wave-free ratio compared with fractional flow reserve, suggesting it has a poor correlation with FFR and that it should not be used in clinical practice. Instantaneous wave-free ratio (iFR), the adenosine-free index of lesion assessment, had been proposed as an index of stenosis severity that is independent of hyperemia. In the ADVISE study, investigators led by Justin Davies, MD, PhD, of the Imperial College, London, reported that iFR had comparable diagnostic efficiency to FFR for the identification of obstructive coronary lesions (ie, FFR <0.8). These two new studies sought to further examine the utility of iFR in comparison with FFR.

Children with HIV are at an increased risk for lipid abnormalities and insulin resistance, and vigilant monitoring of these abnormalities is necessary to provide treatment, according to findings published online recently.Rohan Hazra, MD, of the Eunice Kennedy Shriver National Institute of Child Health and Human Development at NIH, and colleagues published data on 249 children in Latin America with perinatal HIV. The researchers looked at the children’s insulin, lipid and glucose levels.
