Hospitals that provide uncompensated care in states that do not offer expanded Medicaid access may face an additional burden in 2014 as a result of cuts to Medicaid under the Affordable Care Act, according to a study published in the New England Journal of Medicine.U.S. hospitals that are paid funds from the Medicare and Medicaid disproportionate share (DSH) will receive a 75% across the board cut to the DSH under the Affordable Care Act. However, if their state does not expand Medicaid access, who becomes covered in each state will vary widely, stated John A. Graves, PhD, in a news release.

Preinfarction angina associated with significant myocardial protection for stenting

A retrospective analysis and the largest cohort to date found that preinfarction angina is associated with significant myocardial protection for primary PCI with stenting during STEMI.“Preinfarction angina is very common (20%-40% of patients), but is never taken into account in clinical trials that are examining ways to reduce infarct size or reperfusion injury. These patients should be accounted for in clinical trials because they may have a significant impact on the data,” study investigator Jay H. Traverse, MD, with Minneapolis Heart Institute Foundation at Abbott Northwestern Hospital, Minneapolis, told Cardiology Today’s Intervention.

Angina not associated with CV events, death in diabetic patients

A post-hoc analysis of the BARI 2D trial found that patients with type 2 diabetes, stable CAD and documented myocardial ischemia were at similar risk for CV events and death regardless of whether they had angina, angina equivalents or were asymptomatic.The analysis included 2,364 patients with type 2 diabetes and documented CAD enrolled in the BARI 2D trial. The cohort had 1,434 patients with angina, 506 with angina equivalents and 424 with neither. All patients had suitable coronary anatomy for randomization into either PCI or CABG strata.

Advisa MRI pacing system receives FDA approval

Medtronic today announced that the FDA approved its second-generation Advisa DR MRI SureScan, which was specifically tested and approved for use with MRI scanners in the US.The Advisa MRI pacing system features include complete automaticity with Ventricular and Atrial Capture Management to eliminate manual threshold checks and extend longevity of the device; diagnostic and management tools to assist in the early detection and treatment of atrial fibrillation; remote monitoring by Medtronic's CareLink Network; rate drop response; and high upper tracking rate to provide support at higher heart rates for active and younger patients.

CBO lowers estimate for 10-year freeze of payments under the SGR

The Congressional Budget Office recently released its Budget and Economic Outlook: Fiscal Years 2013 to 2023, which lowered the estimate to hold current payment rates under the sustainable growth rate for 10 years from $243.7 billion to $138 billion. “We welcome the news from the [Congressional Budget Office] CBO that the cost of permanently replacing the flawed Medicare physician payment formula, known as [sustainable growth rate] SGR, has decreased dramatically,” AMA President Jeremy A. Lazarus, MD, stated in an AMA press release. “The new cost of freezing payments for 10 years is $138 billion, more than $100 billion less than the previous projection. The rate of Medicare spending growth declined compared to historical trends, and spending for physician services affected by the SGR is projected to be far less than previously estimated.”
